Eyelid surgery is among the most requested procedures in Thailand and across Asia, but many people are unsure whether they need double eyelid surgery, lower blepharoplasty, or both.
Different eye concerns call for different solutions. Choosing the wrong one costs money and time, or worse, leads to revision surgery. This article sets out the difference so you can match the procedure to the problem.
What is double eyelid surgery?
Double eyelid surgery creates a crease in the upper eyelid, making the eye look larger, more defined and more awake. It is one of the most popular procedures in Asia, particularly among those with a monolid or an indistinct crease.

Non-incision (suture) method
Fine sutures are passed through small openings to form the crease, without a long incision. Recovery is quick — three to seven days — with minimal scarring and little discomfort. It suits people whose eyelids are not thick, without excess fat, who want a fast result.
The crease can, however, loosen in roughly 10–30% of cases over five to ten years, depending on skin quality, weight change and self-care.
Incision method
An incision is made along the intended crease, which lets the surgeon remove excess tissue, fat or muscle at the same time. The result is more permanent and suits thicker eyelids, more fat, or anyone wanting long-term durability. Recovery takes one to two weeks, with the final result at one to three months.
Will a sutured crease come undone?
This is asked constantly. It can, particularly in those whose weight fluctuates, whose eyelids are thick, or who rub their eyes frequently. The incision method is significantly more durable, and a lost crease can be corrected surgically.

What is lower blepharoplasty?
Lower blepharoplasty corrects the area beneath the eye: protruding fat pads, loose skin, or hollows that make the face look tired and older than it is. The result is a visibly fresher, more rested appearance.
It is generally performed from age 30 onwards, since under-eye bags usually arise from age-related tissue changes — though those with a strong hereditary tendency may have it earlier.
Approaches
External approach — suited to those with both fat pads and excess skin, both of which can be addressed in one operation.
Transconjunctival approach — the incision sits inside the eyelid, leaving no visible external scar. It suits those with fat pads but still-firm skin, typically younger patients.
Will the bags come back?
With good general health, adequate rest, sun protection and no smoking, results can last five to fifteen years or more — though the underlying ageing process continues naturally.

Recovery
- Double eyelid, suture: three to seven days, minimal swelling and bruising, quick return to normal life

During recovery, avoid rubbing the eyes, avoid strong sunlight, avoid smoking and alcohol, and sleep with the head elevated to reduce swelling.
